what is the external breast prostheses reimbursement program?

The Australian Government provides a reimbursement of up to $400/24 Months for each new or replacement external breast prosthesis for women who have had breast surgery as a result of breast cancer. This applies to women who have had either a mastectomy, lumpectomy or breast-conserving surgery.

Should you require a new prosthesis within the 2 year period, due to a cancer treatment or other medical condition, you may be eligible for a further reimbursement with GP letter.

HOW TO CLAIM IN 3 STEPS:

One

Make an appointment at Leila O'Toole in Sydney for a fitting
by SMS 0403095371
or calling 02-9231 1705.
On the day of your fitting you will be able to purchase your prosthesis and take it home with you.
We will provide you with a printed Medicare claim form. 
If you provide your email address we will send you a reminder in 2 years time.

Return your completed form in one of the following ways:
  1. ON-LINE Scan and email the completed form and receipt to: [email protected]
  2. POST to: Department of Human Services, External Breast Prostheses Reimbursement Program, GPO Box 9822 in your capital city
  3. IN PERSON: Take the completed form and receipt to your local service centre.

Three

You will receive your reimbursement via Electronic Funds Transfer (EFT) to the bank account nominated on your claim form.
Examples: If you purchased a single prosthesis for $429.95 your reimbursement will be $400.
If you have had a bilateral mastectomy and have purchased 2 prostheses for $800, your reimbursement will be $800.
If you have had breast-conserving surgery and purchase a partial breast form for $142, your rebate from Medicare will be $142.
